PRN Registered Nurse - Orange, CT
About The Position
Constellation Health Services is seeking a compassionate and skilled Registered Nurse to provide hospice care to patients and their families in homes, assisted living facilities, and skilled nursing facilities. This is a PRN/per diem position in Orange, Connecticut. The organization emphasizes clinical excellence and compassionate, respectful care in community settings. The role includes providing skilled nursing care, coordinating patient care with other disciplines and caregivers, monitoring and reporting changes in patient status, participating in interdisciplinary team meetings, and completing clinical documentation.
Requirements
- Graduate of an accredited RN program.
- Current Registered Nurse license in the state of employment.
- At least 1–2 years of RN experience.
- Basic knowledge of the iOS operating system.

- Previous hospice care experience is strongly desired.
Responsibilities
- Provide skilled nursing care, including administering medications and treatments prescribed in the physician's plan of care.
- Perform comprehensive patient assessments and coordinate the plan of care with involved disciplines, family members, and primary caregivers.
- Observe and evaluate patients' physical and emotional status and report changes to the nursing supervisor and attending physician.
- Participate in weekly Interdisciplinary Team Meetings and provide updates on patients' physical, emotional, psychosocial, and spiritual status.
- Maintain and process documentation in compliance with agency standards.
- Use a computer-based documentation system and submit clinical documentation, time records, and other required information within required time frames.
- Provide input to the Home Health Aide Supervisor as needed for hospice aide evaluations.
- Provide weekend and holiday coverage as needed.
