About The Position

Activate Care is hiring a community-focused Registered Nurse to travel to members’ homes and community settings across Massachusetts to perform in-person health assessments for dually eligible members of a managed care health plan. The role focuses on timely, accurate assessment and documentation, followed by handoff to the health plan’s care management team; it does not involve developing care plans or ongoing case management. The role is field-based and requires 70% or more travel within the assigned Massachusetts area. Nurses work independently, using the health plan’s care management platform, GuidingCare, and sending SOAP notes to assigned Care Managers. Primary hours are daytime slots based on member availability, with optional evening or weekend scheduling depending on shift selection. The posting welcomes both full-time and part-time applicants, while active onboarding is currently focused on full-time positions. Candidates who complete the interview process and accept an offer by November 30 are eligible for a $2,000 sign-on bonus upon successful hire and onboarding. Employment requires an annual influenza vaccination during flu season, subject to applicable accommodation provisions. The company does not sponsor work visas at this time.
