About The Position

Activate Care is hiring a community-focused Registered Nurse to travel to members’ homes and community settings across Massachusetts to conduct in-person health assessments for a managed care health plan’s dually eligible members. The role focuses on timely, accurate assessments and documentation, followed by handoff to the health plan’s care management team; it does not include care plan development or ongoing case management. Primary work hours are during daytime slots based on member availability, with optional evening or weekend scheduling depending on shift selection. The posting welcomes both full-time and part-time interest, although active onboarding is currently focused on full-time positions. Activate Care’s mission is to improve health equity and outcomes. Its Community Care Record platform, Care Link, helps healthcare and community organizations coordinate care for populations with health-related social needs. Path Assist is its technology-enabled Community Health Worker program for addressing those needs through a structured, evidence-based intervention.
