About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Registered Nurse guides patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care, provides education, coaching, and treatment-decision support, and collaborates with a Care Advocate and Social Worker. The role includes a $15,000 sign-on bonus for external candidates. The nurse works to promote patient health and independence, coordinate with the care team, supervise LPNs and HHAs, and complete timely and accurate documentation. The position also includes assigned on-call responsibilities, staff education, and participation in clinical development and continuing education programs.

Requirements

Current RN license specific to the state(s) assigned for work.
At least 1 year of clinical experience as a Registered Nurse.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines based on identified patient needs, or document the rationale for not making referrals.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
Supervise LPNs and HHAs.
Complete documentation timely and accurately.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
