About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Registered Nurse role supports patients in receiving care at home and builds trusted relationships with patients and families throughout their health journey. The posting describes the role as a Nurse Case Manager who guides patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care, provides education, coaching, and treatment-decision support, and collaborates with a Care Advocate and Social Worker. The role also includes supervising LPNs and HHAs, providing staff education, and performing assigned on-call responsibilities.

Requirements

Current RN license specific to the state(s) assigned to work in.
Current CPR certification.
At least 1 year of clinical experience as a Registered Nurse.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Refer patients to other disciplines based on identified needs, or document the rationale for not doing so.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
Supervise LPNs and HHAs.
Complete documentation accurately and on time.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
