About The Position
Adoration Home Health is seeking a dedicated Home Health Registered Nurse to provide patient-centered care in Milledgeville, Georgia, serving Baldwin, Putnam, Hancock, Wilkinson, and Washington counties. The role provides one-to-one care in patients’ homes, with independent work supported by an interdisciplinary team. The schedule is Baylor/weekends, with flexibility in scheduling. The nurse assesses patients’ physical, emotional, and psychological needs; develops care plans aligned with physician orders and patient goals; delivers nursing care and symptom management; and educates patients’ families and caregivers. The role also includes documentation and participation in the local branch’s on-call rotation as required.
Requirements
- Associate or bachelor’s degree in nursing from an accredited college of nursing.
- Current unrestricted registration and nursing license in the applicable state.
- At least one year of nursing practice in a patient care setting.
- Valid driver’s license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
- Current CPR certification.
Nice To Haves
- Experience in home health, geriatrics, or another related setting.
- New nursing graduates may be considered in select markets based on program availability.
Responsibilities
- Assess and monitor patients’ physical, emotional, and psychological needs.
- Create home health care plans aligned with physician orders and the patient’s goals.
- Direct nursing care, including administering medications, treatments, and interventions.
- Provide pain and symptom management.
- Educate and support patients’ families and caregivers.
- Collaborate with an interdisciplinary team.
- Maintain accurate and timely documentation.
- Participate in the local branch’s on-call rotation as required.
