About The Position

Adoration Home Health is seeking a dedicated Home Health Registered Nurse to provide patient-centered care in patients’ homes. The role is based out of the Milledgeville, Georgia office and covers Baldwin, Putnam, Hancock, Wilkinson, and Washington counties. The schedule is Baylor/weekends, with flexibility in scheduling. The nurse works independently while receiving team support and provides one-to-one care to patients and families.

Requirements

• Associate or bachelor’s degree in nursing from an accredited college of nursing.
• Current unrestricted registration and nursing license in the applicable state.
• One year of nursing practice in a patient care setting.
• Valid driver’s license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
• Current CPR certification.

Nice To Haves

• Experience in home health, geriatrics, or other related settings is preferred.
• New nursing graduates may be considered in select markets based on program availability.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create home health care plans aligned with physician orders and patient goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the local branch’s on-call rotation as required.
