Home Health Baylor Registered Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence at home. Working with physicians and other clinicians, you manage care plans that support patients’ recovery. In this Home Health Weekend Baylor Registered Nurse role, you provide skilled nursing care in patients’ homes, develop and implement individualized care plans, monitor patient conditions, educate patients and families, maintain accurate care records, coordinate with healthcare professionals, and oversee Home Health Aides and their documentation.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.
Nice To Haves
- One year of nursing experience.
- Home Health experience.
Responsibilities
- Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and families about disease management, medications, and treatment options.
- Maintain accurate patient care records and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee Home Health Aides and monitor appropriate completion of their documentation.
