Home Health Baylor Registered Nurse
About The Position
As a CenterWell Home Health nurse, you will report to the Clinical Manager and provide personalized, one-on-one care to help patients regain independence in their homes. You will work with physicians and other clinicians to manage care plans that support recovery. The role provides skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. It also involves developing and implementing individualized care plans, monitoring patient conditions, educating patients and families, maintaining care records, coordinating with healthcare professionals, and overseeing Home Health Aides. A $10,000 sign-on bonus is available.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

- One year of nursing experience.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families about disease management, medication, and treatment options.
- Maintain accurate patient care records and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee Home Health Aides and monitor their appropriate completion of documentation.
