Activate Care•Cambridge, MA
•$69,420 - $104,000•Onsite

About The Position

Activate Care is hiring a community-focused Registered Nurse to conduct in-person health assessments for a managed care health plan’s dually eligible members across Massachusetts. The role is field-based and requires travel to members’ homes and community settings. The nurse evaluates members’ physical, cognitive, and functional status, documents assessment findings, and hands them off to the health plan’s care management team. This position focuses on assessment and documentation rather than care plan development or ongoing case management. The nurse records findings in the health plan’s GuidingCare platform, sends SOAP notes to assigned Care Managers, and submits complete documentation within 48 hours of each assessment. The role requires independent field work, adherence to safety and mandated reporting requirements, and compliance with HIPAA, NCQA, and other applicable regulations. The position involves 70% or more travel within the assigned Massachusetts area. Primary hours are daytime slots based on member availability, with optional evening or weekend scheduling depending on shift selection. Activate Care’s mission is to improve health equity and outcomes. Its Care Link platform enables healthcare and community organizations to coordinate care for populations experiencing health-related social needs, while its Path Assist program uses Community Health Workers and structured interventions to address those needs. Full-time and part-time opportunities are advertised, with onboarding currently focused on full-time roles. Candidates who complete interviews and accept an offer by November 30 are eligible for a $2,000 sign-on bonus upon successful hire and onboarding.
