About The Position

Activate Care is hiring a community-focused Registered Nurse to travel to members’ homes and community settings across Massachusetts to perform in-person health assessments for a managed care health plan’s dually eligible members. The role focuses on timely, accurate assessments and documentation, followed by handoff to the health plan’s care management team; it does not involve care plan development or ongoing case management. The position is field-based and requires 70% or more travel within the assigned area. Candidates may apply for full-time or part-time opportunities, although onboarding is currently focused on full-time positions. The nurse conducts assessments of members’ physical, cognitive, and functional status, documenting results in the health plan’s GuidingCare platform and sending SOAP notes to the assigned Care Manager. Documentation must be submitted within 48 hours of each assessment, and audit or review findings must be resolved within two business days. The role also requires adherence to field-safety procedures, mandated reporting laws, HIPAA privacy rules, NCQA standards, other applicable regulations, and mandatory training requirements. Visits take place in varied residential environments and may involve changing weather and extended periods of sitting, standing, or driving. Primary working hours are during daytime slots based on member availability. Optional evening or weekend scheduling may be available depending on shift selection, though the posting also states there are zero weekend requirements. An annual influenza vaccination is required during flu season, October 1 through March 31, with proof due within 30 days of hire; accommodation requests are evaluated under applicable law. Candidates who complete the interview process and accept an offer by November 30 are eligible for a $2,000 sign-on bonus upon successful hire and onboarding. The company does not sponsor work visas.
