About The Position

This role is for a Registered Nurse in a Long Term Care Clinic setting at Seton Lockhart Health Center in Lockhart, Texas. The position is full-time, Monday through Friday from 8 am to 5 pm. The nurse will lead the implementation and continuous monitoring of individualized care plans, coordinate multidisciplinary teams, advocate for patient safety by interpreting physician orders and communicating clinical changes, navigate the discharge planning process, and champion clinical excellence by delivering direct nursing care.

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Cross accepted.
  • Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
  • Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
  • Holds an active Registered Nurse license with background in Internal or Family Medicine.
  • Hands-on proficiency using the Athena EMR platform.
  • Demonstrates strong customer service, problem-solving, and multitasking abilities.
  • Independence to work autonomously.
  • Flexibility to travel as needed.

Responsibilities

  • Lead the implementation and continuous monitoring of individualized care plans to ensure every patient receives safe, evidence-based treatment.
  • Coordinate multidisciplinary teams as the primary point of contact, ensuring seamless communication across all specialties for holistic patient recovery.
  • Advocate for patient safety by accurately interpreting physician orders and communicating real-time clinical changes to the care team.
  • Navigate the discharge planning process in collaboration with the healthcare team to ensure patients have a clear, supported path for their transition home.
  • Champion clinical excellence by delivering direct nursing care that adheres to the highest organizational standards and safety protocols.
