About The Position

The Registered Nurse - Long Term Care will provide services in the long-term care division, which includes three units: long-term care, short-stay, and hospice/palliative care residents. The role involves all aspects of the nursing process, including assessment, planning, implementation, and evaluation of patient care, with consideration for age-specific needs. Responsibilities encompass patient daily living activities, comprehensive assessments, medication administration, IV therapy, interdisciplinary team coordination, managing admissions and discharges, handling internal and external transfers, delegating tasks to assistive personnel, electronic medical record documentation, patient transport, and responding to emergencies.

Requirements

  • Completed courses for nursing from an accredited program approved by either the Commission for Education in Nursing or the Commission on Collegiate Nursing Education.
  • A current license in at least one state, territory, or commonwealth in the US or the District of Columbia.
  • A minimum of three years' direct patient care experience.
  • Must have Basic Life Support certification.

Responsibilities

  • Provide services in the long-term care division comprised of 3 long-term care units: long-term, short-stay, hospice/palliative care residents.
  • Be responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
  • Assess, plan, implement, and evaluate care based on age-specific components.
  • Manage patient activities of daily living.
  • Perform patient assessments.
  • Administer medications and procedures per established policies and guidelines.
  • Provide ion administration and IV therapy.
  • Coordinate with the interdisciplinary team.
  • Manage admissions and discharges.
  • Handle internal and external transfers.
  • Delegate tasks to assistive personnel.
  • Perform electronic medical record documentation.
  • Facilitate patient transport.
  • Respond to emergency procedures.
