Registered Nurse, RN, Home Health Visits
About The Position
BAYADA Home Health Care is seeking a full-time Registered Nurse to perform home visits to clients throughout Summit County, Ohio. The RN is an integral member of a multidisciplinary health care team, providing skilled nursing and rehabilitative care that helps clients remain at home. Care is expected to be delivered with compassion, excellence, and reliability. The role includes assessing patients, recognizing changes in condition, providing acute and post-acute nursing care, responding to urgent clinical situations in the home, reconciling medications, and delivering wound care. The nurse works independently, makes real-time clinical decisions without immediate on-site support, and manages a dynamic visit schedule while documenting care using point-of-care technology. BAYADA offers paid training in the office, in clients’ homes, and online, along with around-the-clock clinical support. Compensation is $53–$63 per point, depending on qualifications and subject to negotiations, with weekly pay and flexible scheduling.
Requirements
- Current nursing license in good standing in the state of Ohio.
- Graduation from an accredited and approved nursing program, documented by school transcript or diploma.
- At least one year of current or recent hands-on medical experience within the past two years with adult or geriatric patients in acute care, emergency department, home health, hospice, ICU, med-surg, sub-acute care, or rehabilitation units in medical or skilled nursing settings.
- Strong assessment skills and ability to recognize changes in patient condition.
- Ability to work independently and know when to escalate concerns.
- Comfort making real-time clinical decisions without immediate on-site support.
- Strong problem-solving skills in unpredictable home environments.
- Ability to manage a dynamic visit schedule and prioritize urgent visits.
- Efficiency with point-of-care documentation and technology.

Responsibilities
- Perform home visits to clients throughout Summit County, Ohio.
- Conduct thorough head-to-toe assessments and identify subtle changes in patient condition.
- Provide acute and post-acute care, including care related to CHF, COPD, wound care, IV therapy, and post-surgical needs.
- Triage and independently manage urgent clinical situations in the home setting.
- Perform medication reconciliation, patient teaching, and safety monitoring.
- Complete comprehensive wound assessments, wound care, and documentation.
- Make real-time clinical decisions and escalate concerns when appropriate.
- Manage a dynamic visit schedule and prioritize urgent visits.
- Use point-of-care documentation and technology efficiently.
- Provide culturally sensitive care while respecting patient autonomy.
