About The Position

Activate Care is seeking a community-focused Registered Nurse to travel to members’ homes and community settings across Massachusetts to conduct in-person health assessments for a managed care health plan’s dually eligible members. The role focuses on timely, accurate assessments and documentation, followed by handoff to the health plan’s care management team. Primary hours are daytime, based on member availability, with optional evening or weekend scheduling depending on shift selection. Annual influenza vaccination is required during flu season under the health plan policy, subject to applicable accommodation provisions. Activate Care’s mission is to improve health equity and outcomes by helping healthcare and community organizations coordinate care for people with health-related social needs. Its Care Link platform supports community care coordination, and its Path Assist program uses community health workers and structured interventions to address those needs.
