About The Position

Activate Care is hiring a community-focused Registered Nurse to conduct in-person health assessments for a managed care health plan’s dually eligible members across Massachusetts. This mobile, field-based role involves traveling to members’ homes and community settings, completing assessments of physical, cognitive, and functional status, documenting findings in the health plan’s care management platform, and handing results off to the care management team. The role focuses on assessment and documentation, not care plan development or ongoing case management. The position requires at least 70% travel within the assigned Massachusetts area. Work involves independent home visits in varied residential environments and exposure to changing weather, with extended periods of sitting, standing, or driving. Primary hours are daytime slots based on member availability; optional evening or weekend scheduling may depend on shift selection. The posting welcomes both full-time and part-time interest, while current onboarding is focused on full-time positions. An annual influenza vaccination is required during flu season (October 1–March 31), with proof due within 30 days of hire; reasonable accommodation requests are evaluated under applicable law. Employment is contingent on a clear driving record and maintaining personal transportation and continuous auto coverage. The company does not sponsor work visas at this time.
