About The Position

Concordia Visiting Nurses is seeking a Registered Nurse to provide home health care in the Pittsburgh area. The organization is a Medicare-certified, state-licensed, Medicaid-approved home health agency serving nine Western Pennsylvania counties. The Home Health RN provides skilled, personalized care and supports patients and families with respect, dignity, and compassion.

The role involves conducting patient assessments, developing individualized care plans with physicians, patients, and families, providing treatments and other clinical care, and educating patients and families to help them manage health conditions and build confidence in self-care. The nurse uses clinical assessment skills to monitor patient conditions and identify changes. The position offers schedule flexibility, autonomy, and collaboration with a supportive team.

Requirements

• A minimum of one year of recent clinical experience as a Registered Nurse is recommended.
• Home care experience is recommended.
• Adaptability, compassion, and a commitment to patient-centered care.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct thorough patient assessments.
• Develop individualized care plans in collaboration with physicians, patients, and families.
• Provide treatments, start IVs, manage chest tubes, and provide wound care.
• Educate and empower patients and families to manage health conditions and build confidence in self-care.
• Use clinical assessment skills to monitor patient conditions and identify changes.
