About The Position

Activate Care is hiring a community-focused Registered Nurse to travel to members’ homes and community settings across Massachusetts and conduct in-person health assessments for a managed care health plan’s dually eligible members. The role focuses on timely, accurate assessment and documentation, followed by handoff to the health plan’s care management team. It does not involve developing care plans or providing ongoing case management. The position is mobile and field-based, with 70% or more travel within the assigned Massachusetts area. Visits involve varied residential environments and may require extended periods of sitting, standing, or driving. Primary hours are daytime slots based on member availability; optional evening or weekend scheduling may depend on shift selection. The posting is accepting both full-time and part-time interest, while current onboarding is focused on full-time roles. Activate Care’s mission is to improve health equity and outcomes. Its Care Link Community Care Record platform supports coordination between healthcare and community organizations, and its Path Assist program uses Community Health Workers to address health-related social needs. Employment requires an annual influenza vaccination during flu season, subject to applicable accommodation processes. The company does not sponsor work visas at this time.
