About The Position
The Registered Nurse plans, coordinates, and delivers quality home health and hospice care to patients in their homes. The role primarily focuses on home health, with some hospice care. The nurse assesses patient needs, develops individualized care plans, provides skilled nursing services, educates patients and families, and collaborates with physicians and the interdisciplinary care team. This is a field-based position serving Bend, Oregon, and surrounding communities, with in-home patient visits. Requirements
- Graduate of an accredited school of nursing.
- Current Registered Nurse (RN) license in the state of practice.
- Current CPR certification.
- Excellent assessment, communication, and clinical skills.
- Ability to work independently and manage priorities effectively.
- Valid driver's license, reliable transportation, and automobile insurance.
- Ability to perform patient-care physical requirements, including lifting, transferring, bending, standing, and walking for extended periods.
Nice To Haves
- Two years of nursing experience preferred.
- Home health, public health, or hospice experience preferred.
