About The Position

Elara Caring is hiring a full-time Registered Nurse for home health. The role provides nursing care in patients’ homes, with a focus on delivering the right care at the right time and place. The nurse assesses patients, provides care according to plans of care and physician orders, monitors changes in condition, and supports patients and families through education. New ambient listening technology is available to reduce documentation time. A $5,000 sign-on bonus is offered.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on their care and safety.
• Take appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
