About The Position

Adoration Home Health is seeking a dedicated Registered Nurse to provide patient-centered home health care in the O'Fallon, Illinois coverage area. The position provides one-to-one care in patients’ homes, with flexible scheduling and the ability to work independently while receiving team support. The nurse assesses patient needs, develops care plans, provides and coordinates nursing care, educates families and caregivers, and maintains documentation. Participation in the local branch’s on-call rotation is required as needed. This is a part-time position. A $10,000 sign-on bonus is offered to eligible external candidates only.

Requirements

• Associate or bachelor’s degree in nursing from an accredited college of nursing.
• Current unrestricted registered nursing license in the state of practice and the state of residence.
• At least one year of nursing practice in a patient care setting.
• Valid driver’s license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
• Current CPR certification.

Nice To Haves

• Experience in home health, geriatrics, or another related setting is preferred.
• New nursing graduates may be considered in select markets, depending on program availability.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create home health care plans aligned with physician orders and patient goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the local branch’s on-call rotation as required.
