About The Position

Amedisys is hiring a Registered Nurse case manager to provide home health care to patients in their homes. The role is scheduled for full-time day work and focuses on helping patients heal at home through assessment, care planning, education, and coordination with the care team. The nurse also supervises LPNs and home health aides, supports staff education, completes documentation, and performs assigned on-call responsibilities.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP of Clinical.
• Current RN license specific to the state or states where the candidate is assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

• Assess patients and collaborate with the care team to develop and implement plans of care.
• Refer patients to other disciplines when indicated by their identified needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation accurately and in a timely manner.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
