About The Position
Health Link Home Health & Hospice is seeking a compassionate, organized Registered Nurse Case Manager to join its San Francisco home health team. The role is suited to experienced home health nurses as well as hospital nurses seeking a change of pace. The nurse manages a caseload and delivers skilled care in patients’ homes, with support from clinical leadership and intake teams. Full-time nurses are paid hourly for scheduled hours, including drive time and documentation, and the position offers flexible start times and days. Mileage reimbursement and a company vehicle may be available based on availability. The employer has served the Bay Area for over 20 years and describes its culture as collaborative and supported by organized scheduling and clinical systems.
Requirements
- Current, active California RN license; applicants must include their license number with the application.
- At least 1 year of professional nursing experience.
- Valid California driver's license and reliable transportation.
- Strong clinical judgment, organization, and ability to work independently.
Nice To Haves
- Home health experience is preferred; strong hospital and skilled nursing facility nurses are encouraged to apply. Home health orientation and OASIS training will be provided.
Responsibilities
- Manage a caseload and perform comprehensive OASIS assessments, including Start of Care, recertifications, and discharges.
- Administer medications, provide wound care including wound vacs, and manage IV therapies.
- Coordinate care as the primary point of contact for physicians and the interdisciplinary team, including PT, OT, and ST.
- Educate patients and families to help them manage their conditions safely at home.
- Complete accurate, timely clinical notes using AI-assisted tools.
