Registered Nurse (RN) Home Health - 10K sign on bonus
About The Position
Health Link Home Health & Hospice is hiring a full-time Home Health RN Case Manager for its San Francisco Peninsula team. The role offers hourly pay for scheduled hours, including drive time and documentation, and a $10,000 sign-on bonus. The position is direct hire, not contract, with flexible start times and days. The company describes its approach as supporting clinician autonomy while providing real-time clinical and intake team support, organized scheduling, and a steady caseload. The RN manages a home health caseload, conducts OASIS assessments, provides skilled nursing care, coordinates with physicians and interdisciplinary team members, educates patients and families, and completes clinical documentation using AI-assisted tools on a company-issued iPad. Rideshare credits are provided for San Francisco patient visits; mileage reimbursement and a company vehicle may also be available.
Requirements
- Current, active California RN license; applicants should include their license number.
- At least one year of professional nursing experience.
- Valid California driver's license and reliable transportation.
- Strong clinical judgment and organizational skills, and the ability to work independently.
Nice To Haves
- Home health experience is preferred; hospital and skilled nursing facility nurses are encouraged to apply. Home health orientation and OASIS training will be provided.
Responsibilities
- Manage a caseload and perform comprehensive OASIS assessments, including Start of Care, recertifications, and discharges.
- Provide skilled care, including medication administration, wound care (including wound vacs), and IV therapy management.
- Coordinate care as the primary point of contact for physicians and the interdisciplinary team, including PT, OT, and ST.
- Educate patients and families to help them manage conditions safely at home.
- Complete accurate, timely clinical notes using AI-assisted documentation tools.
