About The Position

Health Link Home Health & Hospice is seeking a compassionate and organized RN Case Manager to join their San Francisco/Peninsula team. For over 20 years, Health Link has served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. This role is designed for nurses who desire autonomy while still having strong support systems. The position offers a reliable, hourly income for scheduled hours, including drive time and documentation, with no per-visit pay. AI-assisted charting tools and real-time support from clinical leadership and intake teams are provided to help manage caseloads and maintain a manageable pace. This is a full-time, direct-hire position.

Requirements

Current, active California RN license (required). Please include your license number with your application
Minimum 1 year of professional nursing experience (required)
Valid California driver's license and reliable transportation (required)
Strong clinical judgment, organization, and the ability to work independently

Nice To Haves

Home health experience preferred
Strong hospital and SNF nurses are encouraged to apply, and we'll provide home health orientation and OASIS training

Responsibilities

Perform comprehensive OASIS assessments, including Start of Care, Recertifications, and Discharges
Administer medications, provide wound care (including wound vacs), and manage IV therapies
Serve as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to keep the plan of care on track
Empower patients and families to manage their conditions safely at home
Complete accurate, timely clinical notes using AI-assisted tools
