About The Position
Health Link Home Health & Hospice is seeking a compassionate and organized RN Case Manager to join their growing San Francisco team. For over 20 years, Health Link has served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. This role is designed for nurses who desire autonomy and a stable, full-time income. Nurses are paid hourly for all scheduled hours, including drive time and documentation, with no per-visit pay. The position offers a growing patient census for a steady schedule, AI-assisted charting on company-issued iPads, and real-time support from clinical leadership and intake teams. This is a direct hire, full-time position.
Requirements
- Current, active California RN license (required). Please include your license number with your application.
- Minimum 1 year of professional nursing experience (required).
- Valid California driver's license and reliable transportation (required).
- Strong clinical judgment, organization, and the ability to work independently.
Nice To Haves
- Home health experience preferred.
- Strong hospital and SNF nurses are encouraged to apply, and we'll provide home health orientation and OASIS training.
Responsibilities
- Perform comprehensive OASIS assessments, including Start of Care, Recertifications, and Discharges.
- Administer medications, provide wound care (including wound vacs), and manage IV therapies.
- Serve as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to keep the plan of care on track.
- Educate patients and families to manage their conditions safely at home.
- Complete accurate, timely clinical notes using AI-assisted tools.
