About The Position

Haven Hospice is seeking a dedicated On-Call Registered Nurse to provide hospice care in patients’ homes and facilities in Chiefland, Florida, and surrounding counties. The schedule is full-time: Friday from 4 p.m. to midnight, and Saturday and Sunday from 8 a.m. to midnight. The RN provides after-hours care and support to hospice patients and their families, responding to urgent needs with compassion and clinical judgment. This role offers independent work with team support and an opportunity to make a meaningful impact in end-of-life care.

Haven Hospice provides hospice services tailored to the needs of patients and families, with a focus on delivering care in the home. The position is subject to a Level 2 background screening under Florida requirements.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an automobile that is insured according to state or organizational requirements and in good working order.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Provide hands-on hospice care to patients and supportive services to their families during after-hours on-call periods.
• Assess and manage urgent symptoms promptly and compassionately.
• Administer medication or adjust care in accordance with hospice protocols or provider orders.
• Communicate changes in a patient's condition to the physician and obtain orders as needed.
• Guide and support patients and families in understanding the current condition and anticipating next steps.
• When a death occurs during on-call hours, go to the home or facility to pronounce death, support the family, contact the funeral home, and notify the hospice team.
• Complete timely documentation and communication.
