About The Position

VitalCaring is hiring a field-based Hospice Registered Nurse to provide patient- and family-centered end-of-life care in Metairie and Covington, Louisiana. The role is for weekend on-call coverage from Thursday through Sunday. The nurse serves as a clinical leader, advocate, and guide, delivering personalized comfort-focused care aligned with each patient’s goals and wishes. The position involves working autonomously while collaborating with an interdisciplinary team and supporting patients and families through emotionally meaningful moments.

Requirements

• Active RN license or compact eligibility.
• Graduate of an accredited nursing program.
• At least 1 year of clinical nursing experience.
• Strong clinical judgment and a compassionate approach to care.
• Ability to independently manage a caseload.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Nice To Haves

• Experience in hospice, palliative care, or home health.
• Strong pain and symptom management skills.
• Case management and interdisciplinary care experience.
• Ability to quickly build trust with patients and families.
• Experience with Homecare Homebase (HCHB).

Responsibilities

• Conduct comprehensive patient and family assessments.
• Develop and manage individualized, comfort-focused care plans.
• Deliver skilled nursing care focused on pain and symptom management.
• Serve as case manager and coordinate care across the interdisciplinary team.
• Collaborate with physicians, social workers, chaplains, and aides.
• Monitor patient condition and adjust care plans to maintain comfort.
• Provide education, emotional support, and guidance to patients and families.
• Help families navigate complex decisions with empathy and clarity.
• Build trusting relationships with patients and families.
• Maintain accurate and timely documentation.
• Participate in interdisciplinary team meetings and care planning.
• Support quality and compliance standards.
