About The Position

Citizens Memorial Hospital is recruiting a full-time Registered Nurse for its Hospice service. The RN provides compassionate, patient-centered care focused on comfort, dignity, support, and maximizing quality of life for patients and their loved ones. The role involves visiting patients in homes and other care settings, coordinating with an interdisciplinary team, and supporting patients and families through changes in condition and difficult conversations. The work shift is weekends, and the position participates in on-call rotations.

Requirements

• Associate’s degree or bachelor’s degree in nursing.
• Current Registered Nurse (RN) license.
• Strong patient care and communication skills.

Responsibilities

• Review schedules and coordinate care for hospice patients.
• Travel to patients’ homes, assisted living facilities, nursing homes, and other care settings.
• Perform comprehensive nursing assessments and monitor symptoms.
• Provide emotional support and guidance to patients and families during difficult conversations.
• Collaborate with physicians, social workers, chaplains, aides, and other members of the hospice interdisciplinary team.
• Respond to urgent patient needs and help families navigate changes in a patient’s condition.
• Complete documentation and maintain accurate patient records.
• Participate in on-call rotations to support patients and families.
