About The Position

As a Home Health Registered Nurse, you provide skilled, compassionate care to patients in their homes, helping them recover, manage chronic conditions, and live safely and independently. You use clinical expertise to prevent hospitalizations, empower patients and families, and improve long-term outcomes through trusted relationships and coordinated care. The role serves Spartanburg County, South Carolina, on a full-time evening schedule from 4:30 p.m. to 8:30 a.m., Monday through Thursday. Compensation starts at $46 per visit, with mileage reimbursement.

Requirements

• Graduate of an accredited Registered Nurse (RN) program.
• Active South Carolina RN license or Compact License.
• Minimum one year of nursing experience.

Nice To Haves

• Experience in Home Health, Medical Surgical, Critical Care, or Case Management.
• Familiarity with OASIS documentation.

Responsibilities

• Provide skilled, patient-centered care in patients’ homes in partnership with physicians and the interdisciplinary team.
• Perform comprehensive patient assessments, including accurate OASIS documentation.
• Develop, implement, and adjust individualized plans of care.
• Monitor patient condition and promptly communicate changes to physicians to help prevent complications.
• Educate patients and caregivers about medications, treatments, disease management, and home safety.
• Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care.
• Prepare patients and families for safe, successful discharge.
• Accurately document care in the EMR.
