About The Position

As a Home Health Registered Nurse (RN), you provide skilled, compassionate care in patients’ homes to support recovery, chronic condition management, safety, and independence. The role focuses on preventing hospitalizations, empowering patients and families, and improving outcomes through trusted relationships and coordinated care. You will work with physicians and an interdisciplinary team to provide individualized, compliant care and prepare patients for safe discharge. The position is full-time, with an evening shift listed as 4:30 p.m. to 8:30 a.m., Monday through Thursday, serving Spartanburg County, South Carolina.

Requirements

• Graduate of an accredited Registered Nurse (RN) program.
• Active South Carolina RN license or Compact License.
• Minimum one year of nursing experience.

Nice To Haves

• Experience in Home Health, Medical Surgical, Critical Care, or Case Management.
• Familiarity with OASIS documentation.

Responsibilities

• Provide skilled, patient-centered care in the home while partnering with physicians and the interdisciplinary team to support safe recovery and long-term health.
• Perform comprehensive patient assessments, including accurate OASIS documentation.
• Develop, implement, and adjust individualized plans of care.
• Monitor patient condition and promptly communicate changes to physicians to help prevent complications.
• Educate patients and caregivers about medications, treatments, disease management, and home safety.
• Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care.
• Prepare patients and families for safe, successful discharge.
• Document care accurately in the EMR.
