About The Position

Amedisys is hiring a Registered Nurse case manager to provide home health care. The role supports patients in healing at home through assessments, care planning, education, and coordination with the care team. The nurse also supervises LPNs and home health aides, completes documentation, communicates patient progress, provides staff education, and handles on-call duties as assigned. The stated pay range is $35.60–$44.60; the posting does not specify the pay period.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines based on identified patient needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
