About The Position

Lehigh Valley Companion Care Homes is seeking a compassionate and dedicated Registered Nurse (RN) to provide high-quality in-home and community-based care, supporting clients’ health and well-being in a comfortable and nurturing environment. Requirements

Hold a current, valid Registered Nurse (RN) license in Pennsylvania.
Be able to work independently and manage patient care responsibilities.
Use a compassionate, patient-centered approach to care.
Have strong communication skills and work effectively with clients, families, and healthcare professionals.
Be able to meet the physical demands of the job, including lifting and moving patients as needed.
Be able to work part-time on an as-needed (PRN) schedule.

Nice To Haves

At least one year of nursing experience in home care, long-term care, or a similar healthcare setting is preferred.

Responsibilities

Provide skilled nursing care to patients in their homes and community settings.
Administer medications and monitor prescribed treatment plans.
Perform health assessments, including taking vital signs, and document findings.
Assist with personal care tasks as needed.
Support patients with mobility and physical therapy exercises.
Ensure patient safety and comfort through appropriate care and supervision.
Coordinate with family members and healthcare providers to support optimal care.
Respond to medical emergencies and provide first aid as needed.
Document care provided and maintain accurate, current patient records.
