About The Position

Suncrest Hospice seeks a Registered Nurse (RN) to provide care and services to hospice patients in their residences, which may include private homes, assisted living facilities, and hospitals. The RN completes hospice admissions and patient visits as needed, answers calls from patients, families, and customers, and makes nursing visits for symptom management, deaths, and admissions. The position requires advocacy for hospice services and Suncrest Hospice, along with strong clinical and communication skills. This is an after-hours on-call role working seven days or nights on, followed by seven days or nights off.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state or states of practice.
• One year of experience under license.
• Strong clinical and communication skills.
• Ability to work collaboratively within a team environment and independently on a day-to-day basis.
• Ability to communicate by phone, email, and in writing, and to meet with a variety of healthcare professionals.
• Understanding of hospice rules and regulations.
• Time-management skills to coordinate visits, documentation, meetings, and urgent needs.
• Ability to learn an electronic medical records system.
• Willingness to drive, reliable transportation, a valid driver's license, and auto insurance.

Responsibilities

• Provide care and services to a group of hospice patients in their residences, including private homes, assisted living facilities, and hospitals.
• Complete hospice admissions and patient visits as needed.
• Answer calls from patients, families, and customers.
• Make nursing visits for symptom management, deaths, and admissions as needed.
• Advocate for hospice services and Suncrest Hospice.
• Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Manage visits, documentation, and meetings; respond to urgent needs by prioritizing and triaging multiple concerns.
• Travel across the service area to visit patients.
