About The Position

Greater Springfield Senior Services, Inc. (GSSSI) is hiring a Registered Nurse for its State Home Care Program in Springfield, Massachusetts. The role involves home visits throughout the program’s catchment area to provide personalized care and nursing assessments to consumers in their homes. The nurse also completes paperwork and computer work to maintain accurate documentation. This position combines direct consumer interaction, care planning, and administrative responsibilities in support of older adults, caregivers, and people living with disabilities.

Requirements

• Current Massachusetts RN license.
• At least one year of clinical experience and/or nursing experience in a community or long-term care setting; alternatively, a diploma RN may qualify with two additional years of community-based nursing or long-term-care direct-service experience.
• Community nursing proficiency.
• Ability to prepare and maintain nursing documentation in paper and electronic formats.
• Strong communication skills.
• Strong organizational abilities.

Responsibilities

• Conduct home visits throughout the program’s catchment area.
• Provide personalized care and nursing assessments to consumers in their homes.
• Develop comprehensive care plans tailored to each consumer’s needs.
• Prepare and maintain nursing documentation in paper and electronic formats.
• Complete paperwork and computer work to document consumer care and progress.
• Collaborate with team members and families to support consumers.
