About The Position
Mass General Brigham Home Care is seeking a Registered Nurse to join their Paul Revere team. This role offers the opportunity to build meaningful relationships, use clinical judgment, and make a difference in patients' homes. The position provides independence in managing patients and schedules, supported by a strong team. The role involves working with a diverse urban patient population in areas like Dorchester, Jamaica Plain, Mission Hill, and Roxbury. The team offers a supportive environment with dedicated Admission RN support, ancillary services, a strong team culture with regular meetings, and opportunities for growth through ongoing training and education. The onboarding process is designed to prepare nurses for independent work in the community, with dedicated time for documentation and access to an electronic documentation system. Safety resources, including GPS-enabled tools and real-time alerts, are provided for field work.
Requirements
- Graduate of an approved School of Nursing
- Current Massachusetts Registered Nurse license
- At least one year of acute care nursing experience
- Comfort working with various computer systems
- Strong interpersonal and customer service skills with the ability to work with diverse populations
- Willingness to travel throughout Mass General Brigham Healthcare at Home’s geographic area
Nice To Haves
- Home care experience preferred
- Experience with handheld clinical devices preferred
Responsibilities
- Oversee the patient’s plan of care
- Work closely with an interdisciplinary team
- Complete patient assessments and admissions
- Develop and manage individualized care plans
- Provide skilled nursing care, education, and treatment
- Coordinate care with patients, families, and providers
- Document care and review charts
- Collaborate with the interdisciplinary team
