About The Position

This Registered Nurse role uses clinical knowledge to assess clients and create practical care plans without providing direct or physical patient care. The RN conducts scheduled, non-clinical visits in clients’ homes, speaking with clients and families, observing their needs, reviewing health information, and developing individualized plans for non-medical in-home caregivers to follow. The work emphasizes assessment, communication, documentation, care planning, and relationship-based support rather than clinical tasks or procedures.

The nurse also serves as a clinical resource to the care team and collaborates with the office team to support care delivery. Visits are scheduled, and the role is suited to nurses seeking to move away from physically demanding, shift-based clinical environments while continuing to use nursing judgment and supporting clients and families.

Requirements

• Active Registered Nurse (RN) license.
• Ability to complete thorough verbal and observational assessments.
• Strong documentation and care-planning skills.
• Comfortable working independently in the community.
• Professional, compassionate communication style with clients, families, and caregivers.

Responsibilities

• Travel to clients’ homes for scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, functional abilities, and daily routines.
• Identify risks, support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers to follow.
• Serve as a clinical resource to the care team regarding the client’s plan of care.
• Maintain accurate and timely documentation of assessments and care plans.
• Collaborate with the office team to support care delivery.
