About The Position

The RN Case Manager manages patients’ care plans from admission through discharge and ensures the delivery of quality patient care. The role combines direct and indirect patient care activities, directs staff nurses, LPNs, and CHHAs in delivering each patient’s plan of care, identifies interdisciplinary needs, and coordinates allied health clinicians. The position covers DeSoto County, Mississippi, and is paid on a per-point basis; the stated annual compensation is an estimate.

Requirements

• Graduate from an approved school of professional nursing.
• Hold a current license to practice as a registered nurse in the state where the agency operates.
• Have at least one year of experience as an RN.
• Hold current CPR certification.

Responsibilities

• Assess the nursing needs of assigned patients and manage their overall care plans.
• Complete initial patient assessments upon admission, including obtaining appropriate patient histories.
• Select appropriate nursing diagnoses.
• Assess medications for potentially ineffective therapy, adverse reactions, significant side effects, drug allergies, and contraindications.
• Establish realistic, measurable, observable goals consistent with each patient’s diagnosis.
• Inform physicians of assessment results and recommended clinical interventions.
• Implement nursing interventions consistent with the patient’s diagnosis and established goals, within the scope of the Nurse Practice Act.
• Re-evaluate patients’ nursing and other clinical needs as required.
• Document and submit assessments, progress and discharge notes, and other required paperwork in a timely manner.
• Inform physicians and other personnel of changes in patients’ conditions and needs.
• Direct staff nurses, LPNs, and CHHAs in delivering patients’ plans of care; identify interdisciplinary needs and coordinate allied health clinicians.
