About The Position

Activate Care is hiring a community-focused Registered Nurse to travel to members’ homes and community settings across Massachusetts to complete in-person health assessments for a managed care health plan’s dually eligible members. The role focuses on conducting timely, accurate assessments and documenting findings for handoff to the health plan’s care management team. It does not involve care plan development or ongoing case management. The nurse conducts required Uniform Core, Functional, and Comprehensive Assessments; documents findings in the health plan’s GuidingCare care management platform; and sends SOAP notes to the assigned Care Manager. Documentation must be submitted within 48 hours, with audit or review findings resolved within two business days. The nurse works independently in the field, follows safety and mandated reporting requirements, and complies with HIPAA, NCQA, and other applicable requirements. This mobile field-based position requires 70% or more travel to members’ homes and community settings within the assigned Massachusetts area. Primary hours are daytime slots based on member availability, with optional evening or weekend scheduling depending on shift selection. The job posting welcomes full-time and part-time candidates, though current active onboarding is focused on full-time positions.
