About The Position

Amedisys is hiring a Registered Nurse to provide home health care and serve as a case manager. The nurse assesses patients, works with the care team to develop and implement plans of care, and helps patients improve their health and independence through teaching and appropriate rehabilitative measures. The role includes supervising LPNs and home health aides, maintaining timely and accurate documentation, communicating patient progress, providing staff education, and completing on-call duties as assigned. Flexible scheduling is available, including a Baylor-weekend option. A $15,000 sign-on bonus is offered. Compensation is listed at $72,000–$89,000 and may vary by geographic market, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical experience as an RN must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other disciplines as indicated by identified needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation accurately and in a timely manner.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
