Registered Nurse Home Health -Sign on Bonus $15K
About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. In this role, the Registered Nurse guides patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care, provides education and coaching, supports treatment decisions, and collaborates with a Care Advocate and Social Worker. The position serves the Fayetteville, North Carolina area and offers a $15,000 sign-on bonus. Compensation includes pay per visit point for visits and hourly pay for certain non-visit activities. The stated annual total cash target assumes full-time employment at 40 hours per week and full productivity. Benefits are subject to eligibility requirements.
Requirements
- Current RN license specific to the state(s) assigned.
- At least 1 year of clinical experience as a Registered Nurse.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other disciplines based on patient needs, or document the rationale for not making referrals.
- Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
- Supervise LPNs and HHAs.
- Complete documentation in a timely and accurate manner.
- Regularly communicate patient progress to the clinical manager and care team.
- Plan and provide staff education.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Participate in clinical development and continuing education programs.
- Perform other duties as assigned.
