About The Position

The Registered Nurse provides age-specific care to an assigned group of patients using the nursing process: assessment, diagnosis, outcome identification, planning, implementation, and evaluation. The nurse collaborates with physicians and other healthcare team members to coordinate and implement procedures and treatments, and uses clinical judgment and leadership skills to coordinate patient care and direct or delegate the activities of the patient care team. The role includes assessing patients’ physical and psychosocial status, identifying care needs and outcomes, developing individualized plans, implementing and evaluating care, administering medications and treatments, maintaining accurate documentation, and supporting patient and family education and discharge planning. The nurse also contributes to performance improvement and directs care team activities as appropriate.

Requirements

Graduate from an accredited School of Nursing.
Must be enrolled in an accredited BSN program within two years of job entry and obtain a BSN degree within five years of job entry.
Current license to practice as a Registered Professional Nurse in New York State.
Specialized certifications as needed.

Responsibilities

Interview patients; explain policies and procedures; review charts; answer questions; assess physical and psychosocial health, pain, mobility, sensory deficits, skin condition, coping, comprehension, and cultural or ethnic needs; and communicate and document findings and care plans.
Identify patient problems, teaching needs, expected outcomes, and individualized goals; develop outcome criteria and establish priorities and activities to achieve the expected outcomes.
Coordinate the cost-effective use of supplies, equipment, and medication; document plans of care; and collaborate with physicians and other healthcare team members.
Implement and reassess plans of care; maintain patient safety; administer prescribed treatments; use patient-care equipment correctly; provide emotional support; perform procedures safely and efficiently; document patient condition, treatment, activity, and responses; respond effectively to emergencies; and contribute to a calm, restful environment.
Communicate with patients’ families or caretakers; participate in discharge planning; coordinate referrals; provide discharge instructions; and evaluate care outcomes.
Measure and document the effectiveness of care; conduct variance analysis related to outcomes; and design, implement, and evaluate systems to improve unit care.
Collaborate with care team members and physicians on treatment regimens and plans of care; provide direction to other care team members; and delegate assignments using clinical judgment, ensuring timely completion.
Interpret and implement physician-prescribed treatment regimens; assist physicians with treatments and diagnostic procedures; and keep the Patient Care Manager or designee and/or physician informed of relevant changes in patient condition or treatment.
Provide decubitus and preventive skin care, apply simple dressings using aseptic technique, apply warm and cold compresses, perform irrigations, and perform other skin-care procedures as needed.
Administer and document medications safely and correctly; understand medication actions, dosages, side effects, and patient implications; prepare local solutions; and educate patients and significant others about drug and food interactions.
Participate in patient and family education; explain a patient’s condition as indicated; communicate pertinent information to the care team and appropriate agencies; use available teaching opportunities and resources; arrange early discharge planning and appropriate referrals; and evaluate and document the effectiveness of teaching.
