About The Position

Activate Care is hiring a community-focused Registered Nurse to travel to members’ homes and community settings across Massachusetts and complete in-person health assessments for a managed care health plan’s dually eligible members. This field-based role focuses on conducting timely, accurate assessments, documenting findings, and handing them off to the health plan’s care management team. It does not involve developing care plans or providing ongoing case management. The RN completes required assessments of members’ physical, cognitive, and functional status while respecting cultural and linguistic needs. The role requires accurate documentation in the health plan’s GuidingCare platform, prompt SOAP-note handoffs to assigned Care Managers, and completion of submissions within required timeframes. The nurse works independently in the field, follows safety and mandated-reporting procedures, and adheres to HIPAA, NCQA, and other applicable requirements. The position is mobile and requires 70% or more travel to members’ homes and community settings within an assigned Massachusetts area. Visits involve varied residential environments, changing weather, and extended periods of sitting, standing, or driving. Primary hours are daytime slots based on member availability; optional evening or weekend scheduling may depend on shift selection. The posting welcomes both full-time and part-time candidates, while current onboarding is focused on full-time positions.
