Registered Nurse, Virtual Hospital at Home, Baptist Downtown
About The Position
Baptist Health is hiring a Registered Nurse for its Virtual Hospital at Home team. The program enables patients to complete their hospital stay at home using remote patient monitoring, technology kits, and communication devices. Nurses staff a 24-hour command center to monitor patients virtually, while contracted personnel provide in-home care when required. The Registered Nurse provides for patient needs through the nursing process of assessment, planning, implementation, and evaluation, consistent with the organization’s mission. The nurse organizes and supervises care provided by supportive personnel, coordinates care provided by ancillary services, and includes patients and their significant others or families in developing individualized plans of care. This is a full-time day-shift position based at the Hospital at Home Contact Center in Downtown Jacksonville.
Requirements
- Associate of Science in Nursing (ASN) required.
- Basic Life Support (BLS) certification required.
- Licensed Registered Nurse required.
Responsibilities
- Assess, plan, implement, and evaluate nursing care to address patient needs.
- Organize and supervise care provided by supportive personnel.
- Coordinate care provided by ancillary services.
- Include patients and their significant others or families in developing individualized plans of care.
- Educate patients, significant others, and families about disease processes and plans of care.
- Serve as a patient advocate.
- Provide care to patients requiring treatment for medical, surgical, or other conditions, including chronic medical conditions and minor injuries or illnesses.
