Registered Nurse, Nights, Virtual Hospital at Home Contact Center, Baptist Downtown
About The Position
Baptist Health is hiring a Registered Nurse for its Hospital at Home program, based at the Hospital at Home Contact Center in Downtown Jacksonville. The program enables patients to complete their hospital stay at home with remote patient monitoring, technology kits, and communication devices. Nurses staff a 24-hour command center to monitor patients virtually, while contracted personnel provide in-home care when needed. The Registered Nurse provides for patient needs using the nursing process—assessment, planning, implementation, and evaluation—in alignment with the organization’s mission. The nurse organizes and supervises care provided by supportive personnel, coordinates care from ancillary services, and includes patients and their significant others or families in developing individualized plans of care. This is a full-time night-shift position.
Requirements
- Associate of Science in Nursing (ASN) required.
- Basic Life Support (BLS) certification required.
- Licensed Registered Nurse required.
Responsibilities
- Provide for patient needs through assessment, planning, implementation, and evaluation.
- Organize and supervise care provided by supportive personnel.
- Coordinate care provided by ancillary services.
- Include patients and their significant others or families in developing an individualized plan of care.
- Educate patients, significant others, and families about disease processes and plans of care.
- Serve as a patient advocate.
- Provide care to patients requiring treatment for medical, surgical, or other conditions, including chronic medical conditions and minor injuries or illnesses.
- Virtually monitor patients in the Hospital at Home program.
