About The Position

Adoration Home Health is seeking a dedicated Home Health Registered Nurse to provide patient-centered care in Smithville and Woodbury, Tennessee, covering Cannon and Dekalb counties. The role provides one-to-one care in patients’ homes, with flexible PRN scheduling and the ability to work independently while receiving team support. The nurse assesses patients, develops care plans aligned with physician orders and patient goals, provides nursing interventions, supports patients and caregivers, and collaborates with an interdisciplinary team. Participation in the local branch’s on-call rotation is required as needed.

Requirements

• Associate or bachelor’s degree in nursing from an accredited college of nursing.
• Current unrestricted nursing registration and license in the applicable state.
• At least one year of nursing practice in a patient care setting.
• Valid driver’s license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
• Current CPR certification.

Nice To Haves

• Previous home health experience.
• Experience in home health, geriatrics, or other related settings.
• New nursing graduates may be considered in select markets based on program availability.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create home health care plans that align with physician orders and patient goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the local branch’s on-call rotation as required.
