About The Position

Pinnacle Home Care is hiring a PRN Registered Nurse to deliver compassionate, patient-centered care in patients’ homes. The nurse designs and implements individualized care plans, provides nursing care and assessments, monitors patient conditions, and coordinates with clinical colleagues, physicians, discharge planners, patients, families, and community health resources. The role also includes accurate clinical documentation, patient and caregiver education, and compliance with agency policies and quality processes.

Requirements

Current RN license for the applicable state.
Valid BLS certification.
At least one year of experience in home health or a healthcare facility.
Familiarity with Medicare and Medicaid guidelines.
Solid understanding of professional nursing principles and home health documentation.
Excellent communication, organizational, and time-management skills.
Valid driver’s license and access to a reliable, insured vehicle.
Ability to visit homes that may not be wheelchair accessible.
Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
Ability to walk, stand, stoop, and occasionally lift or turn patients.

Responsibilities

Design and implement patient-specific care plans to support recovery and overall health.
Provide nursing care in accordance with each patient’s care plan, including comprehensive health and psychosocial assessments.
Monitor patient conditions and coordinate health promotion, prevention, and direct care.
Maintain detailed and accurate clinical records, including required OASIS documentation.
Communicate with clinical managers and colleagues to support high-quality care.
Train and support patients and their families or caregivers.
Act as a point of contact between patients and community health resources.
Work with physicians and discharge planners to provide health education to patients and caregivers.
Confirm daily visit schedules to coordinate with other staff.
Understand the agency’s QAPI program and participate as required.
Follow the agency’s process for managing and addressing customer complaints.
Uphold patient rights in accordance with agency service agreements and admission policies.
Understand agency policies regarding patient discharge and reasons for discharge.
