About The Position

The Registered Nurse in Foundation Medical Partners’ clinical float pool provides interim professional nursing care to patients under the supervision of Providers and the Director of Nursing. The role floats among multiple family practice sites across Southern New Hampshire to fill short-term staffing vacancies. Float pool staff provide clinical coverage across multiple locations and may carry out care in person, by telephone, and through telehealth.

Requirements

Registered Nurse license in the state of practice (NH, an NLC state, MA, or both).
Associate’s degree in Nursing.
Valid driver’s license.
Current BLS/CPR training.

Nice To Haves

Certification in Ambulatory Care Nursing (RN-BC), CCTM, or a relevant clinical specialty.
Bachelor of Science in Nursing (BSN).
One year of experience.

Responsibilities

Carry out the nursing process in person, by telephone triage, and through telehealth, including assessing patient needs, planning care, intervening, and evaluating progress toward goals.
Collaborate with an interdisciplinary team—including physicians, advanced-practice nurses, physician assistants, and other health professionals—to design and implement plans of care.
Administer prescribed medications, treatments, and vaccines; perform point-of-care testing; and collect and handle specimens within the scope of practice.
Educate patients about self-management, medications, nutrition, health, and wellness in accordance with collaboratively designed plans of care.
Document the nursing process accurately and in detail in the medical record.
Float to multiple practice sites to cover short-term staffing vacancies.
