About The Position

Foundation Medical Partners’ clinical float pool provides interim coverage of staffing needs on a short-term basis across multiple sites. Float pool staff perform clinical functions in family practices throughout Southern New Hampshire. The Licensed Practical Nurse (LPN) provides patient-centered nursing care under the supervision of Registered Nurses, Providers, and the Practice Manager. The role supports clinical operations through patient assessment, care coordination, education, medication administration, documentation, and clinical procedures, collaborating with the healthcare team to deliver safe, high-quality care in person, by telephone, and through telehealth. The position is eligible for an Enhanced Float Pool Differential.

Requirements

Current Licensed Practical Nurse license in New Hampshire or a compact state.
Current BLS/CPR training.
Graduate of an accredited school of nursing, or currently enrolled in an accredited program leading to Registered Nurse licensure and holding a current New Hampshire LPN license.

Responsibilities

Participate in the nursing process under RN and Provider supervision through in-person care, telephone triage, and telehealth, in accordance with organizational standards, the state Nurse Practice Act, and other professional standards.
Perform nursing assessments and interventions and evaluate progress toward defined goals of care.
Participate with the interdisciplinary team in designing and implementing plans of care.
Educate patients on self-management activities, medications, nutrition, health, and wellness based on the collaboratively designed plan of care.
Accurately and thoroughly document participation in the nursing process in the medical record.
Administer prescribed medications, treatments, and vaccines; perform point-of-care testing; and collect and handle specimens within scope of practice.
