About The Position
The Registered Nurse plans, organizes, and directs home care services, drawing on nursing experience with an emphasis on community health education and experience. The nurse uses available resources to plan and direct services that meet the needs of individuals and families in their homes and communities. The role includes assessing patients and families, developing and revising care plans, providing nursing interventions and education, and identifying discharge-planning needs. When assigned by the Clinical Supervisor, the nurse also serves as Case Manager and coordinates care for an assigned caseload. The posting describes this position as per diem and lists per-visit rates; visits take an hour or less.
Responsibilities
- Complete an initial assessment of the patient and family to determine home care needs, including a physical assessment and history of current and previous illnesses.
- Re-evaluate the patient's nursing needs every four weeks.
- Initiate and revise the plan of care as the patient's status and needs change.
- Use health assessment data to determine nursing diagnoses.
- Develop a care plan with goals based on nursing diagnoses and therapeutic, preventive, and rehabilitative nursing actions, involving the patient and family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by the physician.
- Counsel patients and families about nursing and related needs.
- Provide health care instructions to patients as appropriate to the assessment and plan of care.
- Identify discharge-planning needs during care-plan development and implement them before the patient is discharged.
- Serve as Case Manager when assigned by the Clinical Supervisor and coordinate patient care for the assigned caseload.
